If you’re already diagnosed with a qualifying medical condition, you’re one step ahead. This official diagnosis is a key part in the application process.
From there, you’ll need to talk to a doctor who can write medical marijuana certifications.
You can’t just go to any medical professional, though. Here’s who can give you a certification and why it’s important.
What is a medical marijuana certification?
A medical marijuana certification proves that a physician recommends mmj for your condition.
Essentially, it’s a form that states their professional opinion. It shows that they support medical cannabis for alleviating, managing or treating your specific symptoms.
The certification alone won’t let you buy mmj. However, it’s another step closer to receiving a patient card. You’ll need this mmj certification to move forward with the application process.
Who can write me one?
According to the Arizona Department of Health Services (ADHS), there are specific kinds of doctors that can write medical marijuana certifications. These include:
- Allopathic (MD)
- Osteopathic (DO)
- Homeopathic [MD(H) or DO(H)]
- Naturopathic [NMD or ND]
Each of these doctors should have a current, valid Arizona license.
A state-approved certification center can also provide an evaluation, diagnosis and certification.
How do I receive a certification?
The above medical professionals are the only ones who can provide a certification. Therefore, you’ll have to ask them directly.
The thought of asking your doctor about medical marijuana might feel nerve-wracking. This is especially true if you haven’t talked to them about mmj before.
Take a deep breath and try your best. To increase your chances, do your research first. Make an effort to learn about how mmj can improve your symptoms. When you meet with your physician, explain your findings.
By going the extra mile and showing up prepared, there’s a better chance that your doctor will honor your request.
What does my doctor have to do, exactly?
Ask your doctor to fill in the Medical Marijuana Physician Certification on the ADHS website. They’ll have to state your qualifying condition and that you’ll get relief from mmj. Your doctor will also need to sign, initial and date the form.
The ADHS won’t accept certifications in different formats. So, make sure your doctor completes this specific form.
What if my doctor won’t write me a certification?
If your physician won’t fill in the form, don’t fret. There are other options.
You can ask another physician. According to the ADHS, the physician that writes you a certification doesn’t have to be the same one that diagnosed your qualifying condition. If you need help finding one, check out the doctor referral program at Desert Rose.
Again, you can also talk to an mmj evaluator at a state-approved certification center. Just be sure to bring documentation of your official diagnosis.
In either case, try to respect your physician’s decision to turn down your request. They might not be aware of the potential benefits of mmj. Seeking a different doctor will save you time (and stress) in the long run.

Once your certification is ready to go, it’s time to submit the remaining paperwork. The ADHS only accepts the Medical Marijuana Program Application online.
Your patient card will arrive by mail within 10 business days. To protect your privacy, the card will be packaged in a plain white envelope. It will also expire one year from the date of issue.
