In Arizona, individuals need a patient card in order to buy medical marijuana. The annual fee of this card is $150.

However, if you can’t afford a medical marijuana card, there is another option. Qualifying patients on the U.S. Department of Agriculture Supplemental Nutrition Assistance Program (SNAP) can apply for a reduced fee of $75.


Who qualifies for a patient card?

To receive a medical marijuana card in Arizona, you must have one of the following conditions:

  • ALS
  • Agitation of Alzheimer’s disease
  • Cachexia or wasting syndrome
  • Cancer
  • Severe and chronic pain
  • Crohn’s Disease
  • Hepatitis C
  • Glaucoma
  • Multiple sclerosis
  • Seizures and epilepsy
  • Severe nausea

It’s possible to ask for an inclusion if you have a condition that is not on the list.

Additionally, a variety of conditions qualify as chronic pain. Examples include arthritis, back problems, fibromyalgia, stomach ulcers and recurring migraines. If you have chronic pain without medical documentation, you may still qualify. Your primary care physician (PCP) or a medical marijuana evaluator can determine your eligibility.

How do I apply for a patient card?

If you already have a diagnosis, your PCP can give you a recommendation. They can do this by completing and signing the Medical Marijuana Physician Certification form.

If you still need a diagnosis, visit your PCP for an evaluation. They will need to give you an official diagnosis before you apply.

You can also go to a state-approved medical marijuana evaluator if you do not think your PCP will accommodate your request to use medical marijuana. Should you meet with an evaluator, bring documentation of your qualifying medical condition.

Next, you need to register with the Arizona Department of Health Services (ADHS) and fill out the Medical Marijuana Program Application. ADHS only accepts applications online.


SNAP patients can pay a lower fee.

An initial/renewal card costs $150 each year. But if you can’t afford a medical marijuana card, it’s possible to receive a discount.

For patients on SNAP, the annual fee is reduced to $75.

All patients must pay $10 to change or replace the card.


How do I prove that I’m on SNAP?

If you are on SNAP, you can apply for a reduced card during the application process for a patient card. You will need one of the following:

  • Copy of eligibility notice
  • Electronic Benefits Transfer (EBT) card that shows current SNAP participation

This documentation proves that you are eligible to pay the $75 fee.


What do I need to complete the application?

To finish the application process, you will need to submit:

  • Electronic copy of photo identification (state ID or driver’s license)
  • Current digital photograph
  • Electronic copy of completed and signed Physician Certification form
  • Electronic copy of completed and signed Qualifying Patient Attestation Form
  • Visa or Mastercard for payment
  • SNAP documentation, as mentioned above

The ADHS requires that all electronic copies are in PDF format.


What else should SNAP patients know?

Take note of your SNAP certification period and patient card expiration date. This will ensure a smooth renewal process, if applicable.

Your SNAP eligibility notice includes your certification period, or how long you will receive benefits. If you need to stay in the SNAP program, you must take certain steps to recertify before the period is over.

Additionally, your medical marijuana card expires one year after the date it is issued. You must submit a renewal application at least 30 days before it expires. This involves another visit to the doctor, updated documentation and yearly card fee.

By recertifying your SNAP eligibility in a timely manner, you can make sure your benefits aren’t interrupted.
